Service and Rate (rates may change — please confirm)

• 60 minutes, individual therapy — $200
• 90 minutes, individual therapy —$295.

Insurance

I accept insurance and can bill in-network plans directly. You’re responsible for any copay, coinsurance, or deductible. I can also produce a superbill for you to provide to your insurer or need the superbill for tax purposes.

In-network (plans change — please confirm): Blue Cross, Blue Shield, Moda Health, PacificSource, Providence, Regence, and Managed Health Network (MHN). Please confirm before making a decision.

I can also work with out-of-network benefits when applicable.

Payment methods

•  Private pay: Card on file in EMR system.
•  Insurance: Card on file for copays/deductible amounts (in your billing system).
